Arresto Menor

I need to file a complaint Against some officers of our association. I got a certification from the grievance committee, but the chairman who prepared the certification did not state under oath that the parties involved have been invited to participate in the proceedings to settle the dispute but that no amicable settlement was reached, which is required under the Rules of Procedure of the HLURB. I prepared below affidavit for such purpose, please advice if it will suffice,

WHAT IS should be THE HEADING of this affidavit ?

I, "name of complainant", of legal age, Filipino, with residential and postal address at "complainants address" , after being duly sworn in accordance with law, hereby depose and state:
1. I have caused the preparation and filing of the attached Complaint at the Grievance Committee of the "Name of the Homeowners Assocition" ;
2. I was issued the Certification by the Chairman of the Grievance Committee as herein attached but the Chairman did not state under oath that "the parties have been invited to participate in the proceedings but no amicable settlement was reached".
