ask ko lang po ung bc ng sister ko surname ng mother namin ung nakalagay tapos kami panangay ang gamit surname ng father namin...pwede po kaya mapalitan ang surname niya kc ang gamit din niya sa father namin pero sa nso bc niya surname ng mother namin ang nakalagay. paano po kaya iyon mabibigyan ng solusyon? salamat po.

kc need po sa school. maaari bang gamitin ang surname ng father namin khit hindi sila kasal. matagal na kc sila hiwalay pero ang ipinagamit ng lola namin na surname e yung sa father namin. possible po kaya na mabago ung surname niya ung father na lang po namin.

Hi. My name is Camille, or so I thought. When NSO changed their system, and didn't allow the use of the late registered birth certificates, I found out that my original birth certificate had no name. Yes, no first name, no middle name, no surname. All other details were filled out except, of all things, my name. I then found out that I was my mom's " anak sa pagkadalaga" and I think my jerk biological dad not wanting to marry my mom, my mom didn't give me a name yet. Rule, I believe, in hospitals is to register the newborns within 24 hrs. And so the blank name. My mom married a man whom I believed was my dad for 20+ years, and I was registered late and used the surname of my "new dad". I have no idea how I would be able to fix this. I believe this is a rare case as I have been told and I can't find answers online. Another problem is the dad I grew up with whose surnmae I used for all my life is already deceased, so he can't just like adopt me and legalize everything. I don't know what to do and the fact that I need my birth certificate now because I'm getting married soon makes me problematic about all these. Hope someone can help. Thanks. P.S. Good day again, Atty. I have problem with my first born's Birth Ceriticate. My husband and I were not married when I gave birth to him so I filed him under my last name but with paternal acknowledgement at the back of the form. We got married when he was in Grade 6 and I filed a change of his last name in our Local Civil Registry. They gave me a copy that it was ammended and they told me that I can show that in my son's school registrar to change his name from all his school records. By the way it was in Marikina, they also told me that I can get a copy of aunthenticated bc of my son with his father's last name after a week... but I wasn't able to get a copy until my son needs it before his graduation in Highschool. When I am trying to get a copy in NSO, I was shocked because it is still under my last name. I tried to go to Marikina LCR and paid the amount that they asked me to pay so they could send the files needed by the NSO. But still nothing happened. and NSO told me that its already administrative case and I need to spend huge amount of money to settle all the corrections needed, which is I think is not fair on my part because I didn't failed to make the changes right after we got married.... is there anything i can do to make it lighter on my part, my son is on his 2nd year in college now and he will need it badly two years from now. I am hoping for your advice and Thank you in advance.

Malaki po naitulong sakin ng website na ito mula ng ma illegal recruit ako... that is why I kept on reading different articles of this site. Now, I would like to inquire as for my child birth certificate, as long time ago I requested an nso copy but was no record on file by my childs name & that I had to produce a late registration, as I did so and used my maiden name as we are not married, I have been using the nso of late reg ever since she studied from elementary till high school, but now entering to college, nso cannot issue the same birth certificate (late registration) as suddenly there is an old birth certificate(the one who the informant was my mother in-law na hilaw) and noted from the birth certificate that we are married which is not. I hope u can help me figure out which is the fastest way I can process being a single mom so I can enroll my kid to college asap. Thank you so much and more power to your site.

I really have a serious problem when it comes to the registration of my birth certificate. Mali po kasi yung surname, parents name even their marriage date registered on my birth cert. We found out na mali pala yung ginagamit naming surname when I was in highschool and since then, ginagamit ko na yung totoong pangalan ng father ko(taiwanese national). Now, I'm at my legal age, I'm having difficulty getting some documents(employment purposes) kaya minarapat ko na din na ayusin ang pangalan ko. I went to the civil registrar office of manila, where they gave a list of requirements to initiate the correction of entry. Ang problema, my father has been deceased since I was just a baby and we don't even have any other documents like his birth cert to present since hindi naman sya dito pinganak at yung death certificate nya e mali din yung naregister na pangalan sa hospital since no one knows the correct spelling of his name back then since taiwanese national sya at hindi rin tuwid magtagalog. And then pumunta ako ng PAO to seek legal advice pero they just turned me down kasi kulang daw ako sa documents(fathers birth certificate) which is really impossible for me to get since wala naman kami contact sa family nya in Taiwan. I went to the embassy of Taiwan, at katakot-takot na lakad ang ginawa ko at pakikipagusap sa ibat-ibang tao para at least to get any documents to support my request for the correction of entry pero in the end wala pa din ako nakuha any documents from them. What we only have is his old and expired passport(that has his signature on it), Old immigration certificate which was issued on 1984 na nagpapatunay na pwede sya magstay dito sa pilipinas and their marriage contract. Nga pala, we also have his signature on my birth certificate which is in chinese character(someone translate the signature and it shows na iba yung signature nya sa printed english name na nakasulat sa birth certificate ko), sya lang kasi iniinterview ng nurse when my mom gave birth on me, e hindi nman straight yung pagsasalita ng tagalog ng father ko kaya kung ano lang naintindhan nung nurse yun lang din nilagay niya.

I really hope I could get some help regarding my case.
Would be happy as well if there is someone, like a legal expert that I can talk to. I'm starting to lose hope with my situation.

I planning to get my passport but I think I'm going to have a problem regarding to my birth certificate.

1. My parents name don't have middle names.
2. My father prefix "Jr." did not appear on my BC.
3. My mother name was double written makes it unreadable and looks like MERRIAM instead of Meriam.

Can this affect my passport application.

Now nagiging tour escort ako sa mga group namin sa office and I travel several countries na rin. Now gusto ng boss ko sa Japan naman ako ipag tour escort kaso nagkaproblema sa NSO Birth certificate ko naka-blocked na yung SARTE. lagi na lang lumalabas yung Abania. Sa korea nakapunta na ako kasi hindi need NSO Cert pag-apply ng visa. Maayos pa kaya ang birth certificate?

Hi, what do you mean that SARTE has been blocked already? When was the first one (ABANIA) registered? When was the second one (SARTE) registered? Do you still have a copy of your NSO birth cert na nakalagay is SARTE? kahit photocopy only? Yes, that can be resolved, but I have to know more details.

Hi Kaycee,

The solution for that is to file a petition for the correction of your gender in the birth certificate. It is quite easy. But since gender is not a mere typographical error on your birth certificate, the correction will be done through a court proceeding.

hello! may problema ko sa apilido. ang gamit kong apilido eversince ay sa tatay ko pero hindi kasal ang parents ko. pinirmahan lang ng tatay ko yung birth cert ko (late registered). ang problema ko ay nung kumuha ako ng birth cert sa nso this year wala yung birth cert na ginagamit ko. meron akong copy na lumang luma na galing sa civil registry ng manila. yun yung may pirma ng tatay ko. pero iba ang nakuha kong birth cert sa nso ay iba at apilido ng nanay ko ang gamit ko. sa school at trabaho yung apilido na ng tatay ko ang gamit ko. paano kaya ang gagawin ko?

I would like to inquire about the chance of changing the surname of my son. I have a son who is almost turning 2 years old carrying his Dad's surname. I want to change my son's surname into my surname for the following reasons:

1. I am not married with my son's father. I don't have any plan of being with him anymore.
2. He cannot support my son as he does not have a stable job and doesn't seem to have a plan in looking for one and is very dependent to his parent which I find difficult in establishing a good provision system to my son especially nowadays.
3. His own name is incorrect; His registered name has been altered by his mom during the time of baptism and never changed it on the local civil registry. He has been using the altered name in all his documents including his acknowledgement of my son and I think it might cause an issue in the future.

My son's father and I lived in together for quite some time to try if things will work out thinking his acknowledgement will lead to building a family. However, because of our irreconcilable differences; I decided for us to part ways. I realized that he is incapable of supporting us -even the baby. It also caused us to grow more apart and now I wanted to completely cut off the ties through my son's last name amendment.

Is there any possibility that I could change my son's surname into my surname? I want to fix his birth certificate as early as possible while my son is not yet going to school. I don't want my son to use his Dad's surname when he go to school.

I hope you could give me advise about what to do. What are the legal steps that I should do?

Kung sakaling live pa ito, magandang problem-solving ito.

1. Anong petsa ng registration na nakuha mo sa NSO?
   Doon sa late registration na sabi mong may pirma ang tatay mo, anong petsa?
   May registration na napick up ang office of civil registrar na inendorse nila sa NSO. Ano ito?
   Dalawa ang registration sa office of civil registrar. Isa lang ang ihohonor. Ito ang nauna.

Ang makakatulong dito ay ang office of civil registrar. Hanapin mo sa records section. Sila rin ang makapagbibigay abiso kung ano ang mahusay na gagawin.
