Hello po. Good day. Ganto po kase yun, since i was a child, ang ginagamit ko pong surname ay Arbiz. Which is the surname of my dad. Requirement po ang birth certificate under the NSO. Pano po kaya iyon? Do I have to change my surname or pwede na po kaya yung magpapaaffidavit na lang ako na yung Arbiz and Nartates ay iisa lang? Thank you po. I really need your help.

Hi,

Before answering your questions, I assume that your parents are not married.

Regards,
Atty. Katrina

They were married po. Isa pong Arbiz at isa pong Nartates.

Naging komplikado with 2 BCs. Kung dalawa ang birth certificate, the 1st will be honored. One has to be cancelled through a judicial procedure.

Good day po! Thank you po. I need your help because naguguluhan rin yung pamilya ko..
