Surname Problem

Hi, please check the following steps:

the father has to file the necessary application with the civil registry of the place where the child was born together with the supporting documents, including:

Certified true copy of the birth certificate of the child;
Affidavit to use the lastname of the father;
Valid Identification Card (ID) of the parents or of the registrant (if 18 years and above);
Additional documents showing proof of filiation in cases where the name of the father was left blank inthe birth certficate of the child.

Once the application is approved by the concerned civil registry, the lastname of the father can thereafter be used by the illegitimate children.

Yung proceso ay ang pagpalit at paggamit ng apelyido ng tatay ng anak. Pero illegitimate pa rin ang status dahil hindi kayo kasal. Kung sakaling ikinasal na kayo, may procesong pag update ng status sa Office of Civil Registry.

simula po nagkaisip aq at nag aral Astibe po ang ginagamit q na apelyedo.yan po ang apelyedo ng biological father q.xa rin po ang kinalakhan q na ama..pero ng kumuha po aq ng nso b.cert ang lumabas pong apelyedo ay Hugo.apelyedo po yan ng unang asawa ng mama q..nun q lang po nalaman na hndi po aq nkasunod sa apelyedo ng tatay q..ngaun po ang lahat ng credentials q ay Astibe pati po baptismal cert..patay na po ang mama q at yung una nyang asawa.yung tatay q na lang po ang buhay.may 2 na po aq anak at nkasunod po sa Astibe ang middle name nila.. balak na din po nmin magpakasal kaso nalilito po aq ngaun kung alin ba ang ggmitin q apelyedo..
ano po ang possible na solusyon d2 sa problem q po?sana po mapayuhan nyo po aq...maraming salamat po..

Sa mata ng office of civil registrar, hindi naman siya komplikado. Pumunta ka sa Office of the Civil Registrar sa City/Town Hall kung saan naregistro ang birth certificate mo at magtanong kung paano mo mapapalit ang iyong surname. May mga dokumento kang ihahanda para mapatakbo ang procedure na ito.
