WHAT WILL BE THE REQUIREMENTS TO CHANGE A LAST NAME ON BIRTH CERTIFICATE?

Good day everyone..i have a friend na sobra ilang years na hindi padin naaayos ung bc niya, i am so concern kasi hindi kami makaalis for abroad dahl dun.. i am wondering kung bakit inabot ng ganun katagal, feeling ko walang nag aayos so ako na ung gumagawa paraan para maayos,..

sa bc niya it was his mother's last name, reason is because his parents were not married. although last name ng mother nia nasa bc niya eh nung nag school siya gamit niya ung sa father niya..and now gusto niya papalitan ung last name niya at ipalagay yung last name ng father niya.. hindi siya makapag work dahil dun..magkaiba ung last name niya sa records niya vs sa birth certificate niya.., so i would like to know anu ano po ung mga dapat gawin at requirements para mapadali ung pag aayos ng bc niya... maraming salamat sa lahat!!
