Good day!

i think your best move will be to write a letter for request for reconsideration.

Hi just want to find out if i can still file for maternity benefits if i will not use my married name for my second child. I am married but has been separated for 5 years now, my sss records has my married name on it, right now im 2 months pregnant and i will not use my married name when i deliver. Is it still possible to file any maternity claims in this case? Please advise thanks!
