Transfer of Land Title from deceased grandfather to the grand child residing in the land

Hi atty,

I need your advice about the transfer of land title from the name of my deceased grandfather to the grandchild who has been residing on the lot for more about 20 years.

These are the conditions

a. the land where my family resides for about 20 years is still under the name of my deceased grandfather.

b. the land was acquired by my father as his share from my grandfather's land.

c. my father is also dead.

d. the land doesn't have its own title. The land is still part of the mother title under my deceased grandfather.

e. how can i separate the land from the mother title and transfer it under my name. I am the grandson..

Please help me..

Is there other heirs who are claiming the property? i am referring to the siblings of your father? are they all still alive?
