we cannot find our Title at Registry of Deeds

Goodday!

I have a problem my mother's Land title is lost and we want to get a new one we go to the registry of deeds in cabarroguis and they cannot find it we have a Declaration of real property tax from the Municipal Hall but when the Title number is searched at their records(hard copy) different name appears with the same Title number. what should we do. can you please help us on how to handle this i have no idea. Thank you very much.
