Arresto Menor

I need advise. I gave birth last 1990 and sa birth certificate ang nakalagay is yung surname ng biological father kahit di kami married. I got married at a later date and since then ang ginagamit ng anak ko sa lahat ng documents ay ang married surname ko.How do we go about correcting this?
