Arresto Menor

year 2000 when NSO cannot find my birth certificate, they advise me to apply for a late registration using my maiden name. 2010 when I apply for a passport and there is no problem on my birth certificate, now 2013 the original birth certificate came out under my father's surname. what do i have to do in this case which I have used my mom maiden name on most of my documents?
