Hi,I would like to apply my mother for Phil Health. unfortunately, i am confused what surname she has to use. she had two husbands and was both married to them. the first wedding was a civil wedding and the second one was church wedding. she is currently using the surname of her 2nd husband. i am not sure if that is right? please give me a legal advice for this. thank you.

for your purposes, i think you should use the same one as yours.
