Arresto Menor

HI, Last name ng anak ko ay naka apelyido sa tatay.
Sya ang informant sa Birth Certificate at naka sign din sya ng affidavit of acknowledgement/ paternity sa likod.
Hindi kami kasal ng father ng anak ko,
3 years old na sya and hindi nabigay ng sustento at hindi rin nagpapakita na.

Posible pa po ba na mabago last name ng anak ko at sa akin na lang na last name anfg gamitin nya???
