Withdrawal of Dead SSS Pensioner

hi atty, ask ko lng po on what to do para maacquit sa if ever kaso or claims that the sss could file on us. Meron po kc transfer of fund sa bank ng sss pensioner husband transfer to her wife acct.?the husband was dead on the time of the withdrawal but unfortunately the manager allow it and signed at the aproval portion and witness on the thumbmark as the signature of the husband ,as a general rule dapat po 2 officer who would sign the aproval portion but i didnot sign it on the date of the transaction. 3 days after the manager ask me to sign on the slip worried that she will be audited. i was forced to sign but upon signing i put a date kung kelan ko cya sinign. meron po b maikakaso sakin sss? wat are the steps i can do to acquit on this unauthorized withdrawal? pls help on this atty
