I will be applying for a passport for my son,he is illigitimate and is using my surname (father). His mother put married on his BC but we were not. I would like to ask if this will cause a problem on his passport application and what adjustments should be done,Thanks.

they may look for your marriage certificate. it will depend on the docs examiner who checks your docs.

i don't agree that the grandmother is necessary, but i would recommend that the father files a petition to have him declared the custodian of the child.
