birth certificate tampered for passport application

good day!
Last march 2014 i applied for a passport & submitted my altered NSO birth certificate. After 2 days, i got a call from DFA stating that my passport application status is pending due to tampered middle name. I was so nervous that time, so I hang up. Eventually I processed my NSO birth certificate correction at LCR and later passed it for NSO authentication.
Question is, can I still apply for a passport? Do I get charged for falsification of public docs?
