maling entry ng lastname

ask ko lang po my husband surname is JUSTINIANI since eto na ung ginagamit nya from the day he study till now but nagkaproblem siya nung recently kumuha siya ng birth certificate sa NSO,ang nakaregister na surname nya is REYES and wlang nkarecord na JUSTINIANI sa NSO,anong process po ang gagawin nya since lahat ng credentials and record nya is JUSTINIANI plus pati kami ng 2 kids namin is using JUSTINIANI...i ask his mother pano nangyari ang sabi is misunderstanding between sa umayos ng birth certificate..however ung umayos sa cityhall is dead na, pano po namin maayos ung birth certificate nya from REYES to JUSTINIANI?

do i need pa po ba nadumaan sa court? and pano po ung process nun at requirements to file?
