Last name issue

Hello po. I need your legal expertise po regarding my last name. Sa lahat po ng IDs ko and school and employment records, I've always used my father's name po. Pero sa NSO Birth Certificate ko po, Mother's last name ang apelyido ko. My parents were never married. I had a municipal form of live birth before na Father's last name ang last name ko so yun ang naging basis for using my last name, tinanggap namam ng school records at govt agencies as long as may affidavit of two disinterested persons. Kaso I was informed di po yun iaaccept for passport application. Im planning to just align my last name po sa nasa BC ko. Pano po ba process? Where do I start?

you would have to have all of your ID's and record corrected to reflect kung ano yung nasa BC mo. iisa isahin mo puntahan yung mga ahensya na concerned para magrequest ng correction at sila ang magaadvice sayo ng steps kung pano macocorrect yung record mo sa kanila.

I don't think so unless you still have some sort of ties with them. In regards to your COE from them, you can just explain to your prospective new employer your situation.

magkakaproblema ka lang kung may discrepancies sa mga requirements mo. halimbawa, your surname on your ID is different from the one specified on your BC.

Hello po. Same case din po yung sa akin. Per married po parents ko. Hndi po ako makapagpa-legitimize kasi po yung name ng father ko sa birth certificate is walang JR. sa ibang papers po, meron. Ipapa-correct ko po sana yun pero nalaman ko po na wala syang birth record sa PSA at sa LCR ng Lupon, Davao Oriental. Naikasal sila ng mother ko dati dahil friend ng Papa ko ung judge. Wala na din po way para maipa-late register dahil wala na po sanpilipinas papa ko at wala na sila relatives sa davao. Nandto po kami sa la union. Ano po kaya dpat ko gawin at may way pa po ba para magamit ko surname nya? Need din po para sa passport. Thank you so much!

Unfortunately, kakailanganin mo ayusin sa LCR yung registration ng BC ng tatay mo before ito mairegister sa PSA.

What if po yung surname nlng ng mother ko ang gamitin ko, pwede po ba yoon kahit married sila?

Hi po. Paano po process para magamit ko legally surname na mother ko na nsa bc ko. May way po ba or kasulatan na pde gamitin bilang patunay na yung nsa mga records ko at bc ay iisa. Para po sna di ko na papalitan lahat. Thank you.
