I have a quick inquiry. My current company is asking me to submit an affidavit of separation so that I can enroll my kids as my dependents for my hmo, I'm separated but not legally. As rule of hierarchy my wife and my eldest will be my dependent. How can I get an affidavit of separation?

you just need to draft one stating the fact that you are separated from your spouse and have it notarized. l
