Hi! Since birth, until I'm grade 3. I'm using the name that appears in my NSO. Then my mother married another man and they changed my last name. I didn't know said course what happened if they really filed it or not. Now my school records, SSS, Philhealth everything except my NSO is using that last name. How will I fix it. Or which will be the easiest. To use my NSO name or the name i've been using for years. Please advise.

It will be easier to switch back to you NSO name. Each of those agencies may have different requirements, but most of them will have to comply with the law

to change your name, you will have to file a court petition.
