My live birth and NSO certificate shows different last name

Ask ko lang paanong paraan ito masolusyonan, live birth ko correct entry ng last name ko which is Sanjuan pero sa NSO Hagmok nkalagay which is Middle name ko dapat yan. Ngayon po ang sinusunod ko pong apelyido is Sanjuan. Married na po ako ngayon ang nkalagay padin sa Marriage certificate na middle name is Sanjuan, sinusunod ko yong nasa live birth ko. pa help naman po thanks
