Arresto Menor

Paano po ba gagawin ko? At kailangan na records para maicorrect yung surname ko. Nakalagay sa birthcertificate ko CORTEZ ung sa parents ko data tama naman ORTEGA walang data sa birthcert ko na pagkukunan nila ng cortez. Ung CORTEZ di nanggaling sa parents ko as in wala kaming kamag anak na cortez. Obviosly mali talaga yun ng gumawa. Kailangan ko pa po ba ng affidavit? Saka ung mga record ko sa school ok lang ba ng isang record lang kukunin ko? Thanks..
