I'd like to seek legal counsel on what to do with my surname.
My parents got married in 1988 (civil) in Manila. I was born in 1989 of April in a hospital in Manila.
All my life, I thought that my name is Marvin A. Hermosa. Because that's what I've been using since elementary and high school.
But to my surprise, when I started working and had to get an NSO birth certificate, my name was Marben Anilag. Anilag is my mom's maiden name. I found this out last 2013.
Some of my ID's are issued in my supposed to be real name Marvin A. Hermosa. and the later ID that I got are Marben Anilag.
As of now, my parents are not anymore living together. He is in Luzon Area, and we are in Cebu.
I asked my mom how it happened. The nurse in the hospital was the one who asked my mom for my name, she mispronounced it, or the nurse misheard it (i really don't know which is true) the nurse did not also ask her to write it down. She also blames my father for not following up about my name. and I also blame her because she also did not follow up about it. It's kind of unfair because my younger brother has my father's surname and I don't. I'm now 28 years old.
I'd like to change the name in the NSO Birth Certificate to my supposed to be correct name.
Please let me know for what actions and documents I need to prepare.
