Good day Ma'am/Sir,

I would like to ask for advice po, paano ko po maichange yung surname ko? kasi sa NSO ko po is "palanca" sa tatay ko, while sa school records ko sa college is "duguiles" paano ko po maichange? kasi kailangan ko po talaga sya maiprocess kasi ito na lang po yung hadlang sa akin para makatake ako ng board exam sa civil.. lumapit na po ako dto kasi wala ako malapitan.. wala kasi willing tulungan ako kahit sa family ko.. kasi anak sa labas kasi ako ng tatay ko, ayaw nila ako tulungan. wala din ako kilala na may alam.. please guys help me Sad Crying or Very sad Crying or Very sad Crying or Very sad

sana matulungan nyo po ako guys. matataray pa yung school registrar namin.. 2years na po ako hnd makatake ng board. masakit isipin kasi yung ganito situation, i feel hopeless.. Crying or Very sad Crying or Very sad Embarassed Sad

Just execute an Affidavit that the person in the birth certificate with the surname Palanca is one and the same person using the surname Duguiles. You might also need two people to say that in a separate affidavit.

It is your records that have to conform to your birth certificate, since it appears you have been allowed to use it even if you are illegitimate. Not the other way around.

Bakit kasi Duguiles ang gimamit eh Palanca naman ang nasa birth certificate?

Or, if you really want to change your surname, you have to file a petition in court.

The grounds however are very specific:(a) when the name is ridiculous, dishonorable or extremely difficult to write or pronounce; (b) when the change results as a legal consequence of legitimation or adoption; (c) when the change will avoid confusion; (d) when one has continuously used and been known since childhood by a Filipino name and was unaware of alien parentage; (e) when the change is based on a sincere desire to adopt a Filipino name to erase signs of former alienage, all in good faith and without prejudice to anybody; and (f) when the surname causes embarrassment and there is no showing that the desired change of name was for a fraudulent purpose or that the change of name would prejudice public interest.

It will cost more money to file this petition and there is no guarantee that your petition will be granted.
