hello,

i need help on what to do with my daughter (eldest) which do not have any CORRECT birth certificate due to the following circumstances: She will be turning 18 this year November

PART 1 OF PROBLEM:

she was born in Manila (Medical Center Manila) Nov 1999 and unfortunately i was not around during the time of birth. the mother my present wife just turned 18 before giving birth that year. when the child was born, the grand mother instructed her daughter(my wife) to put the name of the child as "princess carolina david" and to put the name of the mother as "carolina david jr" then no father, mother is 18yo on the registration of childs birth.. however the real name of the mother who gave birth my wife is maria prinsesa.. and the grand mother's name is carolina without the "jr" so technically all information on the childs registration is INCORRECT.. btw, my daughter grew up never using the "princess carolina david" as her name..

PART 2:

when they came out of the hospital, the grand mother immediately registered the child at paranaque city as (late registration, born "hilot") same year 1999 with the name "princess CAROL david" and under their names as mother & father of the child.. so technically she became the sister of her biological mother Sad but it does not end there.. the registration also indicates INCORRECT information of both parent's age and the mothers maiden name was incorrect due to she was not the informant during the process of the late registration.. our daughter was baptized during that year with the mother/father on her baptismal certificate as her grand parents..

PART 3

we were married 2001, our daughter started school and used "princess carol dellosa" bearing my surname without any proof of birth certificate until she garaduated high school.. now she is in college and needs a birth certificate..

I registered her as "late registration" at Las Pinas City hoping that we can get a correct birth certificate in 2013 unfortunately it did not go thru at NSO because the 1st birth certificate is showing up (Manila Record) the only correct information on her birth certificate(s) MANILA & PARANAQUE is the date of birth..

QUESTION: what do i need or she needs to do so that she can have a correct NSO birth certificate? thanks!
