Termination of Parental Authority

My daughter is bearing her father's surname. But we are not married. He's in the birth certificate with signed affidavit at the back. We were common-law partners (lived-in) until he cheated on me last April of 2016. He moved out from our house by June 2016. He stopped giving financial support on September 2016. He's with someone else now, the same girl he cheated me on. He never visited our daughter since he left. No financial support for almost 6 months now. I have stable work and I am providing for my daughter. He is currently unemployed. I would like to file for termination of parental authority. I would like my daughter to use my surname instead. She is just, 21 months old. How can I do it? What grounds can I use? Thank you.
