Arresto Menor

Hi, I have a problem po with my birth certificate. Showing in my birth certificate po kasi prehas kami ng mother ko ng middle name and last name. Kaya lumalabas po, magkapatid kami. Kailangan daw po baguhin. It's either tanggalin ko ung middle name ko or palitan ko yung last name ko(Illegitimate child), Kukunin ko ung last name ng dad ko. Which one is better po? Tanggalin ung middle name ko? or palitan ko ung last name ko?
