Conditions after resignation

Hi Sir, I am a licensed pharmacist and I have resigned in a distributor company last october. The company's hr have contacted me to get my remaining salary and backpay last week. Upon receiving the cheque, they handed me a piece of paper that have list of conditions such as that I can't apply with their competitors and other companies with the same field, and the other one is that I can't file anything against them. What should I do? Will I will not be able to apply to other distributor companies? Will they sue me if I will apply to other distributor companies?

Yes I have signed it because i was afraid they wont give my remaining salary.

then yes they may sue you if you will apply to other distributor companies.

Does the company have labor code violations? Why would DOLE get involved?

I believe this is a non-compete clause and since you signed it, you have to follow it.
