Ilan beses po ba pwede mag issue ang school ng original form 137?

Kapag na i release na ba ito hindi na pwede mag release ulit ng original?

Paano kung nawala ang original copy nito at ayaw tanggapin ng University ang certified true copy ano ba ang pwedeng gawin?
