I am writing on behalf of my niece po.
May problema po ung NSO record nya.Sa NSO kasi
Lastname nya is family name Ng tatay nya.Pero ever since nun 1yr old sya sa amin na sya tumira kasi hnd responsible tatay.Nung Ng school sya Ang dala nya lastname is family name ng mama nya which is kapatid ko.Ngaun po 21 years old na sya at nkatapos Ng pag aaral.Tapos po ngaun po gusto nya kumuha Ng passport .Problem is ung sa NSO nya lastname hnd match sa school records nya.Patay na po both parents nya.Hindi rin po nya mkuha ung TOR nya dhil dito.Gusto nya po palitan ung lastname nya sa NSO to follow ung school records nya. hnd rin Kinasal parents nya. Hindi ko alam bkit lastname Ng tatay nya nkasulat sa bc at nso.Anu po pwd nmin gawin pra matulungan sya? Salamat po

Hello po,

I have question din po sana re s pagchange ng surname. I wasn't able to read the full topic kc mejo naconfuse ako iba iba dn po kc ng scenarios. Anyhow, ask ko lang po sana. First, yung daughter ko po kasi sa nso nya she's using my last name. but yung first name ko sa birth cert nya iba sa NSO cert ko, gusto ko po sana ipa-correct yun using my first name na nakalagay sa bc ko. Second, i have a son and he is using his fathers' surname gusto ko po sana ipachange yun sa surname ko since he's we were abandoned by him after 6 months kong pinanganak. I would like to ask po sana some legal procedures re doon po sa 2 concerns ko.

Good evening. Paano naman po procedures (including fees) pag mag papacorrect lang ng misspelled surname? Isang letter lang po yung mali. Example: From Dimarucot to Dimarucut.
At what if po mali din yung sa tatay? Do the father needs to correct his birth certificate first?

I just need a legal advise with regards to having my first and last name changed to something else other than my mom and dads family names. I will be using different names po dito as an example so I can better explain my situation and at the same time protect my true identity.

For example, my father registered the name GREG V. MOLINA JR. for me. He put the Jr. there even though Greg is just his nickname. His real name is GEORGE Mandane y MOLINA. My mother's name is EDITH Acosta y MOLINA. They are not married and the reason why they share the same last name is because they are 3rd cousins. They have been separated for about 20yrs now. Now the 'V' there on my middle name stands for VALENCIA. Later when I was old enough to do a little research about my name, I found out we don't have any Valencia in our bloodline.

In other words I have a very fucked up name. Now what I would like to know is is there a way that I can have both of my first name and last name change to something else such as JOSEPH Acosta y MOLINA after my mom, or to an even more personalized name such as JOSEPH Acosta y GUIDO for instance.

The main reason why I'm asking this is because having this very confusing mixed up name seems to be one of the reason why I can't really move forward in life. I honestly can say that I'm not really proud with my name at all. For me it's like a bottleneck to my progress as a person. I just wanted to have my own non-fucked up identity. Is this possible? Thank you.

Hello po!

Una po sa lahat, I'm glad na may ganito palang site that really helps a lot of Filipinos! Kudos to the one who created this!!!:-)

So here's my scenario..
My name is Belle (nickname) and I am 24 y/o that was born and lives in the Philippines. Anak po ako ng isang Retired US Air force Military, pero hindi nia alam na may anak sya dito sa Pilipinas not until mahanap ko sya when i was 22 y/o. Kung di ako nagkakamali, ang first communication namin is June 16, 2015 nung ngreply sya sa snail mail ko through email. He was here in the Philippines in 1991. Nung pinaalis ang mga Amerikano because of Mt. Pinatubo eruption, hindi alam ng mama ko na buntis sya kaya hindi nya nasabi sa daddy ko. Biglaan ang alis nila kaya hindi nakapag iwan ng kahit anong address or contact no. ang daddy ko.. so wala po silang communication at mula non hindi na alam ng mama ko kung nasan ang Daddy ko. Syempre po, nagkaasawa ng Filipino ang mama ko. Sa LCR birth certificate ko po ay step father ko ang nakalagay na father ko, so mula pagkabata ko po ay dala dala ko po ang pangalan ng stepfather ko. Nung kumuha na po ako ng NSO birth cert. nakita ko po at noon lang din nmin nalaman na apelyido ng nanay ko ang nakalagay na apelyido ko at unknown ang father ko. so because i have to take board exam for social worker, inayos ko po iyon para lumabas sa NSO Birth cert. ko na dala kona ang pangalan ng step dad ko, in short (d ko po alam kung tama ako), simulated ang birthcert. ko. Note po na hindi ko pa kilala or wala pa akong communication sa biological father kong Amerikano. Ngayon po na nakilala ko na sya, we are planning na magclaim po ako ng dual citizenship kaso po wala akong maipakita na legal docs na legitimate daughter ako ng biological father ko kasi po sa NSO ko eh name ng stepfather ko ang dinadala ko.. pano po kaya ang pwede kong gawin para maayos ang problema kong ito..

hi good day ask ko lang po anu kailangan gawin kasi yung surname ko first name ko nagkamalimali at di ako gumagamit ng middle initial ever since im a chinese citizen but born here in manila eh nid ko gumawa ng icard kasi requirements sa amin kaso sabi sa immigration nagkaproblema birthcert ko eh kaso nakagawa na ako ng unang icard tapos naka 2 renew na ako tapos ngayun sasabihin nila may problema birthcert ko ang name ko ay chiang min and surname is chen ang nangyari sa birth certh ko yung surname ko chiang name ko chen tapos nilagyan ako ng middle initial na min eh yung min ko kasama dapat yan sa chioang min 2 word name ko pede ba deretso sa nso palitan or dapat sa cityhall tapos pag nagawa na cityhall naba ang magpapadala sa nso ng new certificate?? and ask ko lang how much aabutin yung mga pagawa na yan kasi pinaasikaso ko sa kakilala ko dumeretso kasi siya sa nso mismo tapos napalitan naman yung name pero pagdating sa immigration sabi fraud daw nso ko eh may resibo naman tapos tinatakot ako na pede daw idissmmiss yung registration ko sa icard hindi ba nangiipit yun pede ba sila mag dismiss pero nakapagbayad na ako lahat lahat dun lang sa birthcert ko nagkaproblema hope sana may magreply thanks
