SURNAME CHANGE IN NSO BIRTH CERT

hi, ang problema ko is regarding po sa NSO birth certificate ko. my dad pass away 9 years ago.

1. i am using my father's surname in all my valid documents
2. i am born in UERM q.c.
3. in my NSO birth certificate ang surname na nakalagay is my mother's surname

hindi p ako makakuha ng passport kasi di pa napapalitan yung surname ko sa NSO. ano p ba procedures na dapat gawin at saan dapat lumapit para mapapalitan ang surname ko sa NSO? nagtry po kami sa PAW in q.c. but na decline yung request namin.

Kung ipapabago mo ang apelyido mo sa surname ng dad mo, yung dad mo dapat nun ang gagawa at wala nang ibang makakagawa nun kung hindi siya. Since patay na ang dad mo, unfortunately, hindi na mapapabago ang lastname ng father mo UNLESS you have sufficient evidence na recognized ka ng dad mo na anak niya. If you have sufficient evidence, then kailangan pumunta ka sa court to change your last name.

Another option would be to change all your documents to your mom's surname kasi yun yung nasa NSO birth certificate mo.

Following the option presented by Atty Katrina to you and I assume:
-Gusto mong makakuha ng passport
-May Birth Certificate kang issued by NSO with your mother's surname
-Requirement ng DFA sa 1st time applicants ay NSO issued BC plus other documents

Suggest ko:
1. Kumuha ka ng Philpost Postal ID with biometrics (requirements are NSO BC, Barangay Clearance using your BC name.  At least 2 weeks to process, mailed to you. Cost is P 414.40)
2. File ka ng NBI clearance + Police clearance with Barangay clearance using your BC name (At least 3 days. Cost is P 115.00).
3. Apply ka ng passport under DFA new procedure http://www.dfa.gov.ph/index.php/2013-04-04-07-01-23. Cost is P 950.00 for regular processing in 20 days, P 1,200 if processed in 10 days.

Then work on changing your identification on your other documents.  You will need an Affidavit of Disparity in Name or of Disinterested Persons.

Same problem lang po sa taas. Since bata pa po ako hanggang ngayon, apelyido na po ng father ko ang gamit ko. The difference lang is that sa NSO BC ko, sa first page, wala akong Middle and yung surname po ng mother ko ang nakalagay. May nakaattach na second page for late registration of birth due to late marriage ng parents ko and nakaindicate na po dun yung full name ko with my father's surname. Ngayon po, when I was processing my requirements for work such as SSS, di daw po nila pwede i-honor yung NSO BC ko.According to them, need daw po palitan yung name sa first page na may complete name with my father's surname. Pano ko po papapalitan and ano po requirements para po mapalitan?

By the way, namatay nadin po Dad ko 4years ago and may marriage contract po sila and may baptismal po ako. Can I use those to change my surname? Nasa legal age na po ako to do it. Thank you.

Ang makakatulong sa iyo ay ang office of civil registrar kung saan ka registered. May checklist sila ng requirements sa mga possibilities.

Hindi po ba ako pwede pumunta dumirecho sa NSO para ipabago? Thank you po.

late din po na kasal mga magulang ko, na register po ako sa surname nang mama ko.i think when i was on my early preschool prinoseso na nla dun sa birthplace ko to transfer my surname sa papa ko po. until now yung b.certificate with my papa's surname na ang gnagamit ko, but then nung kumuha ako sa NSO surname pa rin na mama ko ang nakalagay, yung late registered na certificate. ano po ba ibig sabihin?i really need infos coming from you. mgpapakasal na po kasi ako, naabala ang pag.process dahil dito. thank you so much and God bless

Puede kayong dumiretcho sa NSO sa East Avenue pero magbibigay lang sila ng payo. Ang mag aayos ng papeles ay the office of civil registry sa munisipyo o cuidad kung saan naka register. Claiming lang ng NSO certified ang NSO.

Ang office ng NSO na mag eentertain ay National Statistics Office - Central Verification and Enrollment Agency (NSO CVEA) at East Avenue. Sa likod ito ng processing center.

Based on the experience of others, I stand corrected on the use of the PhilPost ID.  Di siya honored sa DFA.  Di rin siya honored as a valid ID sa NSO.  These offices must have reasons for not recognizing it.

My husband to be is also an illegitimate child, his middle and last name came from his mom wala din pong naka state na fathers name nya sa bc nya even acknowledgement. Today, in his legal age, he wants to adopt his fathers surname (willing din po yung father nya), possible pa po ba ito khit my NSO bc na sya? what are the things to do? Hope you'll notice my post! Possible even with the current NSO certified BC. He needs to secure an Affidavit to Use Father's Surname AUSF and file this at the Local Civil Registrar where the BC was registered. There are other supporting documents required like child's consent to use father surname if over 18 years old and evidences to show declared relationship between father and child like insurance, income tax return, SSS etc plus a minimal fee to pay. This will take time. Once you get an updated NSO certified BC, it will not change the front page but an annotation will be stated allowing the use of father's surname.

I assume father is still alive. Check with the Office of Civil Registry for details to validate. Verify with LOCR if a change in surname requires a petition in court. If so, it requires the services of a lawyer to proceed to tedious steps. Bring all the facts with you.

Thanks, Sir Centro. Also, upon presenting of the new NSO pwede npo namin papalitan yung other docs nya like SSS, Philhealth etc.? Same thing din po ba sa pagpalit ng surname ng anak namin?

FYI, Sir. Dulalia po ang last name ng partner ko (his Mother's last name) and yan din po ang dinadala ng mga anak ko. If ever po, maging successful ang change of surname ng partner ko to Contreras (Father's last name) pwede na rin po ba iapply for change of surname yung dalawa kong anak? Para Contreras na po ang dalhin nila? Pls reply po.

hello po, as of this moment po wala kaming lead sa first step na pwedeng gawin for my situation. could you please guide us on the legal procedure na gagawin? or if you can refer us to someone to assist us on this? nagtry na po kami sa PAO navotas, then the lawyer who is very accomodating write a letter to NSO referring us to QC. First step na ginawa na po namin yun Pao qc but unfortunately nirefer lang po kami sa Pao navotas. which is which po ba talaga?

Plan po namin mag file ng PETITION FOR LEGITIMATION AND CORRECTION OF SURNAME, san po ba ang tamang lugar kung san kami dapat lumapit? Pao Qc po ba kung saan ako pinanganak or Pao navotas kung san kami nakatira and home address ko din po?

Hi, pwede po ba magtanong? Yung hawak namin na birth certificate ng anak ko ay surname ng asawa ko pero pagkuha namin sa NSO yung surname ko pa din nkalagay, paano po ba gagawin ?

File it where your record is registered. If approved, the Office of Local Registry will forward it to NSO so master updating. They you secure an NSO authenticated certificate with the updated surname annotated at the back.
