Father's name was not included on the birth certificate

what is wrong? has mothers occupation as house keeper when she was business lady or owner
the address was her mothers not where they lived together,. errors that cant be changed under the ERROR law.
2nd, the father FILED out a COLB before child was born, it was at hospital and it never used,it just disappeared, when father went to hospital they delayed him asked for various requirements even a "cash gift"   ,, then it send to manila civil registry.
they wont assist to allow him to AUSF or acknowledge the birth , the mother will not assist him. at present she is not avaiable,
yet he has care and custody of the child,
what else can you suggest?
CANCEL the 1st?
the COLB must be done right or the embassy will REFUSE the paper work for fathers
acknowledgement for child's other country citizenship

yes atty sa akin ung ginamit n apelyido ng bata, gusto ko sanang ipalagay ung fathers name instead of N/A. ano po b ang pwede kong gawin?

You have to file the necessary application with the civil registry of the place where the child was born together with the supporting documents, including:

1. Certified true copy of the birth certificate of the child;
2. Affidavit to use the lastname of the father (there is a form for this);
3. Valid Identification Card (ID) of the parents or of the registrant (if 18 years and above);
4. For certificates of live birth with unknown fathers, submit additional documents such as the Affidavit of Acknowledgment/Paternity and documents showing father’s signature and other proof of filiation.

Note: In some cities/municipalities, the personal appearances of the mother and the father may be required by the local civil registrar to confirm their identities.

However, since you mentioned that the mother is not available, you can first try going to the local civil registry without her company.

Once the application is approved by the concerned civil registry, the lastname of the father can thereafter be used by the illegitimate children.

Processing time may be more or less a week and the fee is around P1,000.

Hi, yes that is possible. Kinakailangan yung father ng bata ay kinikilala ang baby na anak niya at gusto niyang ipagamit apelyido niya sa baby ninyo.

he does have the Brgy certificate.solo parent of the child, his philhealth w/ child's name is listed Hospital info shows the fathers name on the "medical log" and on the BACK of COLB is the fathers name written by the mother , should this be sufficient? ,
the only change to COLB will be annotated on side of fathers name correct??...

How can 1 obtain a CLEAR and TRUE COLB that doesnt have N/A as father?
cancel the 1st and redo(resubmit) so the COLBis correct..
any recourse to take legal action against the hospital?..

the birth is registered at city of manila.its difficult to get to the city, can this be accomplished out of town or by attorney?
