Arresto Menor

Good Afternoon.

A Notice to Explain was issued to an employee citing an offense on non-compliance to procedures.  During the administrative hearing, another offense was found to have been committed.  May the new offense be cited in the Notice of Decision or a should it be treated as a separate disciplinary case?
