Arresto Menor

We have this employee for 3 years however, shes no longer abiding the company policies. And according sa company policy namen na if suspended for 3 times in a year will lead to dismissal. And as of now she already incurred 6 times of suspension with different offenses. What is the best and legal way to terminate her.
