which birthplace to use?

hey, i'm a graduating student, and i need help which to address to choose on my birthplace. My birth place in the "NSO' is Albay Provincial Hospital,Daraga,albay. that was the old name and address. Now, the hospital's name is BRTTH,Legazpi City. Which will i use? The old one or the new one?!

use the one listed in your birth certificate. the birth place refers to you not the hospital.
