Hello po,
My brother needs his birth certificate to graduate fro mhis school.
Our parents are already dead for so many years. Hindi po kasal ang mga magulang namin. But they lived together for at least 25 years. We all grew up knowing and using our father's last name. On my birth certificate some years ago when I needed the bc for my passport, my name was originally only with our mom's last name (Menor) and added on the back saying I am entitled to use the surname of my father (Alcantara). There is also a remark of my mom's name "also known as ....." My mom used her nickname originally on my birth certificate. So update siya ng birth name niya sa likod ng bc ko po.
Ngayon po ay hinahanap namin ang birth certificate ng kapatid ko. We cannot find it at NSO, pumunta na rin po sa different hospitals na akala namin na ipinanganak siya. Tiningnan na rin na baka same kami ng ospital, we checked sa Binondo where my mother mentioned it at one time. We checked maybe with mom's last name also....we do not know anymore how to move forward. what to do next? Do I need to get a lawyer? I think we already exhausted our options...maybe a lawyer like you can help me how to get a birth certificate for my brother knowing na it seems it does not exist?
