name listed in CMAP

Good Day. I just want to make an inquiry regarding CMAP. I previously have a credit card in Equitable Bank. It is a JCB Card. However my problem is that, it appears that my name appears in CMAP for bad credit while in fact, I dont have any obligations. Legal firm even send me a letter that they'll sue me if I dont settled my accounts which in that time they made the letter. It has been settled 3 months before. I have called them and upon verifying, asked an apology and told me they were not informed that I have already settled my accounts. Equitable bank even send me a certificate or clearance that shows I have already settled my account. An officer in that bank told me that my name was cleared from any records since I have already paid and they had issued a certificate. I believed that but to my dismay I found out that my name is still not cleared in CMAP. Even worse I lost the cerficate issued to me since it was way back 2007. And one more thin is that I cant request for another certificate since Equitable Bank dont exist anymore.

Sir/Madam, how can I possibly cleared my name? I lost the certificate and no more equitable bank to request for another one.
