Is being impregnated by a married man considered ground for concubinage?

Ano ang magiging effect if:
1) the name of the married man is indicated sa birth certificate ng bata as the father, or
2) if the name of the married man is not indicated sa bith certificate ng baby as the father.

or does the wife need first to prove that the incident is "scandalous" per se.

Or kung hindi naman, anong ground ng wife for concubinage?. IF the husband does not cohabit with the other woman?. and does not even keep a mistress in the conjugal dwelling?...
