Dear Sir/Madam,
I am a nurse who are about to start for a new job on a new employer and one of the pre-employment requirements is to submit a copy of the BIR form 2316 coming from the immediate previous employer. My previous employer already given it to me but I lost it. They told me that they no longer have a copy of such as they have given me the original document.
Where should I request for a new copy? Can the BIR provide it for me?
