late registration of NSO birth certificate

hello po atty

may masalimuot na problema po kasi yung birth certificate ko

eto po ang details:

birth date: august 8, 1983
age: 28
hospital: ospital ng maynila

kumuha po ako ng copy sa NSO kaso wala daw po akong record. pinabalik ako sa manila city hall. pagdating ko dun wala din daw silang kopya. punta daw ako kung saang ospital ako pinanganak.

meron naman akong record sa ospital ng maynila

ang nakalagay "august 9, 1983, baby girl + surname ng nanay ko"

kaso di pa daw nila na-forward sa manila city hall kasi wala daw po pirma yung tatay ko nung pinanganak ako. nasa probinsya po kasi ang tatay ko pero kinikilala po talaga nya akong anak. patay na po din sya ngayon.

sabi ng clerk sa OM, pasa lang daw ako ng requirements at sila na bahala mag-forward sa manila city hall tapos po yung manila city hall na ang magpo-forward ng documents sa NSO

kulang po yung requirements ko. wala po akong marriage contract ng magulang ko

di na po ako bumalik sa ospital kasi tingin ko po pag sinabi ko na walang marriage contract, papagamit nila sa akin yung surname ng nanay ko

28 na po ako ngayon

lahat ng records ko college diploma, ITR, employment, NBI, SSS, etc. lahat po surname ng tatay ko gamit ko.

kapag po ba sinabi ko sa ospital na walang marriage contract ang parents ko mapipilitan akong gamitin surname ng nanay ko? isa pa problema august 9 record ko sa ospital.

atty paano po kaya ang maiging gawin?

thanks for the quick response

yes my parents are married

but the date of the marriage contract is the problem

i was born august 8, 1983

my parents had a civil wedding on april 19, 1994

more than a decade after i was born

i worry that date discrepancies will affect my surname

currently, my name in the hospital record is "baby girl Torres"

torres is my mother's maiden name

i'm not sure if the hospital will honor this marriage contract so i can use my father's surname (which i've been using since birth)

if at the time you were born, there was no impediment to your parents getting married, then you should be deemed legitimated and your surname be of your father.

try late registration procedure.
