surname problem. will it need court session?

This is my boyfriend's problem. He uses Figueroa as surname since birth, on his school records as well. However when he graduated in college, he has to provide an NSO copy of his birth certificate and he found out that what's listed there as his surname is his mother's name. After that, his mother told him that his father has a first marriage. His parents are married in civil.

What should he do?

my daughter has been using my surname and my middle name since she entered her school year due to familly matters problem. but when entered her third year in college her university asked her to submit her Birth certificate which is under her father's surname. her school change her name frm my surname to a surname frm her B.C. Now her school send her a letter that she has no record of form 137 and 138 under the name she is using right now.all her documents under my surname has been submitted already b4 she enrolled in first year. what will i do to correct all this asap, because she is graduating next sem.

zhelestine, his father can issue an authority to use surname of the father. he should inquire at the civil registrar where his birth certificate is registered

anna, she should go to her high school and request for amendment of her forms by explaining what is stated on her birth certificate

a lawyer should not be necessary. ask the school what they will require.

My true and correct name should be Ernesto Toriado ORCINE, Jr. which I used and still using, but in my certificate of live birth, it appears to be Ernesto Toriado ORCENE, Jr. Should this be covered by RA 9048? If not, what legal action should I do? Please advise. Thank you!

are your parent's married? is your father still alive? if so, ask him to issue affidavit of acknowledgment of paternity and authority to use surname of the father.
