ask ko lang po kung papano ang process mag pa change ng lastname, ang naka lagay po kasi sa NSo Birt certificate ko na last name ay yung sa mama ko, pero ang ginagamit ko since elementary ay yung last name n g papa ko.

are your parents married? if not, is your father still alive? did he sign the birth certificate?

i have same problem with aristothelcasanova. but the difference is i was born first before my parents got married. and my birth registry was filed late or delayed registration.

first name: john
middle name: (none/blank)
last name: garcia
birthdate: october 9, 1991

mother's maiden name: loida lumbes garcia
father's name: ricardo sales javier
date of marriage: may 24, 1992

date of registration: march 10, 1996

my father signed the birth certificate. the municipal civil registrar issued birth information stating my name with my father's surname, that's why i am using my father's last name since pre-school.

there's no remarks im legitimated and it was indicated on my birth certificate that my parents are NOT MARRIED.

what is the right procedure with my case?

you have to file a copy of their marriage certificate at the civil registrar so that you will be legitimated

I have a problem on my high school diploma. i graduated 1994. wrong spelling ang surname. my husband applied for a job papuntang australia and pinakukuha mga documents ng family nmin. I noticed n wrong spelling pala ang surname ko s diploma. I went to the school where I graduated high school ang sabi hindi n nla ako mbibigyan p ng copy ng diploma dahil wla na yung mga signatories. Certification lang daw ang pwede nilang ibigay. Ask ko lng po,Io-honor po ba ang certification sa pgkuha ng visa going to australia? ano po b ang dapat kong gawin?

pwede ko ba gawin ito sa stepdaughter ko? Father's name sa birth certificate nya is "unknown".
pwede ko ba ilagay name ko don then change surename nya with supporting documents of our marriage contract with her monther?
