good day po.
Gusto ko lang pong malamn if possible na ma change ko yung last name ng anak ko from his father's surname to mine? pagkapanganak ko po sa bata ay apelyido po nang tatay niya ang dala niya. ngayon po gusto kong palitan ito. my son is just 4months old hindi kami kasal.

He's not doing his part as a father. sana po makareply kayo soon.

personal opinyon ko lang po ito (walang legal advice ito). Give yourself muna ng "break o cooling period" ang sarili mo, after that, e assess uli ang feelings mo sa kanya. baka eventually, ay gumagaan uli ang loob mo sa kanya at kayo pa rin ang magkakatuluyan sa huli. para as is na lang yang surname na yan if makakasal kayo.

dahil pag officially registered na yan sa civil registrar at na forwarded na sa Nso ang birth info sa bata, medio hindi simple ang proceso diyan if you wish to change it again. even sa 'correction of entry" ay may mga requirements din.

final advice. punta ka muna sa local civil registrar kung saan naka rehistro ang birth certificate ng bata. kausapin mo ang in-charge dun at hihingi ng opinyon nila. ask if ang "correction of entry" will be its best solution, considering na hindi kayo kasado o legally married, na dapat sana na pwede sa babae ang magdadala sa surname ng bata.
if they say na pwede, then, why not? medio hindi malaki ang gastos if correction of entry lang. pero kung kinakailangang idadaan pa ito sa korte, medio magastos na. pero sa tingin ko, sa local civil registrar mo lang ito e consult for its best solution.

did the father sign the birth certificate? if not, you may have a chance, if he did, there's no legal procedure for it. you will have to file a petition in court, and there's no guarantee it will be granted.
