Good evening po..sana matulungan po ninyo ako sa aking problema tungkol po sa aking birth certificate ang naka registered lang po aking middle name and surname..wala pong pangalan..in short blanko ang name..sa manila po ako ipinanganak,pero lumaki po ako sa dipolog city..ano po dapat kong gawin at eh attached na mga documents na nagpapatunay na itong current name na gamit ko ay since pagsimula ko mag aral ng kinder,elementary up to college,ito gamit ko..til present..thank you po please reply. god bless

You have a file an affidavit of supplemental report with the Local Civil Registrar, attaching copies of your records.
