Arresto Menor

Paano po ba ang gagawin pag nagka baligtad ang Middle name at Surname sa Birth Certificate. Correct naman po ang mga pangalan ng parents ko pero nagkamali ang Civil Registrar sa pag input ng Middle name at surname ko. Please advice

You have to file an administrative petition for correction of entries.

The requirements are with your local civil registrar.
