baptism has no legal bearing.

the law requires you to use the name listed on your birth certificate. yes, you may have problems later on when you try to get a passport, take a prc exam, or any document where your birth certificate is required.

are your parents married? did your father sign your birth certificate?

if you really wish to insist to use your mother's surname, you will have to file a case in court to have your name changed.

cost depends on which laywer you retain. you can approach the PAO, or legal aid office of the IBP or law school. normally it will take 6 mos to 1 year

Can you please give me a rough estimate of how much I will have to spend to file a judicial petition to rectify the typo error in my name and the change in my birthdate. How much would it cost normally to hire a private counsel to undertake this including publication? Marami pong salamat.

it really depends on which lawyer you retain, but P30,000 would be reasonable.

Yung sa birthcert ko po kasi, my nkalagay na (-) in between names (ex. Mary-Ann, pero Mary Ann lang po gamit ko eversince. Would that be considered as typo error? And yung sa birthdate ko po years ang gap nila, how long would it take po ba para maayos yun? Thank you and God Bless!

palmyra, the "-" can be considered a typo error, but not the date of birth.

you can file a petition for correction of the "-" at the civil registrar.

for the date, you will have to file a petition in court to have it corrected.

you can also choose to just use what is reflected on the birth certificate and correct your other records instead.

that would depend on what they require from you. if college records are enough, then stop there. if not, then adjust. good luck.

you have to acquire a marriage certificate of your parents and file with the civil registrar where you birth certificate was registered. You will be deemed legitimated and you can use your father's surname.

As for the birth date, I don't believe they will allow that as a typographical error but you can try. otherwise you will have to file a case in court to correct it or just use that as your birthday.

it will be advisable to change it. use the explanation that it should match your birth certificate.
