Good day Atty. My husband and I were making an online appt para sa passport ng notice namin na mali yung middle name ng mother niya (maiden). Instead na Distor...it was just middle initial B. Nag alala kmi and all, only to find out po na sa birth certificate niya ay the same lang. kakapakasal plang po nami. And we are on the process of visa application (my husband as my dependent). Would you advise to carry on with what is shown in the birth certificate and marriage certificate?

But won't there be a discrepancy later on? What is your husband's full name in your marriage contract? is it FIRST NAME DISTOR SURNAME? And then when it comes to the name of the mother it's FIRST NAME B. SURNAME? There will still obviously be a discrepancy on the face of the document.

You might have to get that corrected because there might be complications later on.

Hindi po ng rereflect sa full name ng husband ko yung error sa maiden name ng mother niya kasi po middle name po ng mother niya ang error sa birth certificate niya. At deceased na po ang mama niya long time ago.

Yes but in the birth certificate itself, the entry under mother's maiden name should be MOTHER'S FIRST NAME MIDDLE NAME (GRANDMA'S MAIDEN NAME) DISTOR, should it not?

That is a discrepancy on the face of the document itself?

Ang Maiden name po ng mother sa birth certificate is
Theresa B. Biter
Ang dapat po sana ay:
Theresa Distor Biter

Yung birth certificate po was used sa PRC at Passport dati.
Na notice lng po namin yung error nung ng fill up kmi ng application for renewal sa DFA.
Sa Marriage certificate po namin exactly the same po ang nakalagay sa maiden name ng mother niya.

But your husband's name in your birth certificate is FIRST NAME DISTOR BITER?

OR

FIRST NAME BITER SURNAME?

First Name Biter Surname which is the correct one.

Ah. Then the error is innocuous. And it seems that it is consistent with the existing records? There might not be a need to correct.

Safe po ba na mag stick nalng kmi sa B (middle maiden name) pag nag fill up po ng forms in the future?

If meron po, you would advice na palitan both the marriage and birth certificate? Hindi po ako sigurado kung anu po nakalagay sa birth certificate...pero pwede po kmi mag request nun?kahit patay na po? And anu po ang valid na other document? Death certificate would do? Alam niyo po ba kung ganu katagal ang process ng change of name?

Because the discrepancy might cause problems later on, like the CEMAR record, it will mention the parties married, and who their parents are.

Yes even when the person is dead next of kin can obtain PSA (formerly NSO) copies of their deceased relative's records.

Chnage of name or correction of entries take a few months.
