good evening po pwede po makahingi ng tulong may first wife po ako then naghiwalay po kami almost 11 years na now then my kinasama po ako at ngakaroon ng apat na anak ,ang panganay ko po sinunod ang name nya sa apelyedo ko pero ng kunin sa nso ang live birth nya is nakasunod surname nya sa misis ko pero lahat ng credintial nya sa school apelyedo ko ang dinadala nya di po siya maka grade8 if di maitama ang birth certificate nya pwd po ba ipacancel ang birth certificate nya at iacknoledge para masunod ang surename nya sa akin salamat po ng marami ayaw ko lng po kasi mapariwara ang anak ko dahil lng dito salamat po

HI,

You have to file the necessary application with the civil registry of the place where the child was born together with the supporting documents, including:
 
- Certified true copy of the birth certificate of the child;
- Affidavit to use the surname of the father;
- Valid Identification Card (ID) of the parents or of the registrant (if 18 years and above);
- Additional documents showing proof of filiation in cases where the name of the father was left blank in the birth certificate of the child.

You and the mother must appear in person at the local civil registry.
 
Once the application is approved by the concerned civil registry, the last name of the father can thereafter be used by the illegitimate children. The fee is Php1,000, more or less.
