Change of Mother's Maiden Name on Birth Certificate and Date of Marriage

I will be applying for a British Passport through decendacy and am required to submit an NSO Philippines Birth Certificate as a required document. A couple of questions with regards to name problems on my birth certificate.

1. Is it required by Philippine law that children retain the maiden surname of their mother as their middle name. My parents did not do this although I have a middle name on my birth certificate which is not my mother's maiden name.

2. On my Philippines Birth certificate my mother's married name has been entered as her maiden name and also the Date of Marriage is incorrect by 2 days, year is correct however. I was born legitimately as my parents had been married for 18 mths before my birth. Can I get these 2 changed. I have my parent's birth certificates and marriage contract showing all the correct names
