Name in Birth Certificate with alias.

Hi Atty. Good Day,

Would like to seek for your guidance regarding my problem in my birth certificate.

In my birth certificate my name has an alias but I never used it since I learn how to read and write. Since grade school up to date, I never use it.

Ex Name shown in my Birth Certificate:

JUAN "DONDON" D. DELA CRUZ JR.

I'm referring to the alias "DONDON".
This causes inconsistency  because all my documents, accounts, IDs, etc., does not have "DONDON" and this is also the reason why I cannot get a passport.

What can I do to fix this?
Does my case needs an attorney?
Or can I just provide an affidavit of same person just to have a passport?
