Hi. I need your help. Since I started schooling I used Xxxx Xxxxxx Xxxxxx. When I was trying to get my college diploma I was asked to provide an authenticated copy of my BC and found out my middle there is "Zzzzzz". My mom used her father's last name when she was single but my lola (the informant on my bc, 'forgot' and volunteered her last name instead w/c is Zzzzzz.

I tried to fix the error on my bc myself through the civil registry but i find it more problematic for the reason that I don't have any complete/correct document to refer to. These are the issues:
1. My older siblings birth certificates have discrepancies on my parents' marriage. On my sis, the day of marriage was Nov 26, 1969 instead of Nov. 28, 1969. My bro birth cert meanwhile got the date correct but the place of marriage incorrect, listed in it is Sta Rita, Samar, instead of Sta. Margarita Samar.
2. I requested a copy of my parents marriage cert from NSO and the copy I got, I cant hardly read my father's name. Plus, on the certificate, both my parents is of same age. But we all know that my mom is 5 years younger than my dad.
3. My mother knows that she was born May 25, 1950 in Sampaloc Manila. When I tried to get a copy of her BC from the civil registry of manila, she has no record.

Anyway, all I ever wanted to to correct my BC in order to get a passport to work afford. Is there any simpler remedy to take for my BC problem? Can you also refer a lawyer who can process my petition to the court?

i have advised persons with similar problem to at least consider using the name on the birth certificate. most have found it easier to convince the school to change the records (example: usc in cebu has a procedure just for this).

it seems you already know that what is required is a petition in court.

Thanks for your reply. Well, I can consider what you said but how about my SSS, TIN, PAGIBIG, Philhealth? They all had been using the middle name Daguman. Will an affidafit suffice in order to correct my records there too?

Hello....My fiancee was going to get a copy of her birth certificate and passport, She found out the the middle name on her birth certificate (Reyes) is not what it should be (Ramos). Her mom gave her a middle name of Ramos but the midwife put down Reyes. All through her life on all id's she used Ramos. Now her birth certificate does not match all her id's. If she uses Reyes, as stated on the birth certificate, can she get a passport even though it conflicts with her id's? Will this cause a problem with the immigration work for me to bring her to the US? Should she get a lawyer to fix the birth certificate? She told me that it will cost $700 USD, which seems pretty high price.

i cannot speak about whether she will have problems with US immigration. USD700 is actually not a very high price, because it takes a court petition to correct her birth certificate. ramos to reyes cannot be considered a typo so a judicial petition is required.

Hi, I have problems here too.

My condition is like this: My birth cert & passport showed middle name "Carenio" but my ID showed " Carino". I want to renew my passport but Department of foreign Affair decline it and ask me for attorney advice. What is the proper procedures and Which is the faster way as I want to go abroad to Malaysia by this Novemeber 2011.

Secondly, how much of currency I can bring out from Philippines to Malaysia? Any limitations?

Hi! I would like to ask about the middle name of my husband.

His correct full name is "Rico Ibarrientos Edroso" but 15 years ago, he applied for a passport to be used in working abroad (saudi arabia) and one of the requirements is the NSO certified birth certificate. He found out that he was registered with a middle name of ibarriNEtos instead of ibarrientos (we still have the copy of it). He did not find time to correct it because an overseas job offer had been given to him. So to make the story short, he used the name written in his birth cert and so his name appeared as "Rico IbarriNEtos Edroso". Same story goes with all his other documents like marriage contract, birth certificate of children, employment contracts/visa.

Last year, we requested again for birth certificate in NSO and it showed up to be corrected without our knowledge. It has been corrected from "ibarrinetos" to "ibarrientos".

Now my question is.. would it be possible to retain the mispelled middle name (ibarrinetos) because our marriage contract and children's birth certificate were based on what is indicated in his passport. We also have pending application to Canada and I am afraid to lose that opportunity because of this issue.

can you recommend a good lawyer for the corrections in my Birth Certificate.

The are numerous wrong entries, first my fathers surname typo instead of M, N was typed. The biggest problem is the nationality, Filipino was typed instead of the nationaly of my Dad.

lanzraniel, autocorrect? this is the first time i've heard of it. there's supposed to be only one recorded birth certificate. could it be that someone made a late registration in an attempt to correct it?

Actually, the correct name is the one which is available now in NSO and the old record seems to be deleted because when we requested last year, it appears first as "negative record" (negative for the middle name "ibarrinetos" but we have a proof of that BC containing ibarrinetos) then we have been asked to go to the civil registrar in Bicol where he was registered and there we found out that it is corrected to "ibarrientos" and no record for "ibarrinetos".

If late registration was made then it would appear in the birth certificate that it is a late registration but it is not and no one from his family applied for a late registration.

I really dont know how to solve this. The civil registrar staff is so snobbish when my sis-in-law tried to fix it (we could not do it personally as we are working abroad).

you can't correct his birth certificate because it isn't an error. it's either he applies for a change of name, or you correct the other docs affected.

Hi! Good day. I have a problem here, too.

I would like to ask, how to correct my mother's middle name po. Kasi nung bata pa sya ang ginagamit niyang middle name ay CARO, ayun yung totoong middle name nya, so definitely lahat ng docu nya CARO ang nakalagay. Then ang nakalagay sa birth certificate ng mama ko ay RIVERA. So there's a big problem po. We're planning to go abroad po sana, pero we couldn't because of the error in her middle name in birth certificate. Could you hepl us? Ano po ang best way? Is it still need the court order? Ayoko na po sanang maglabaws pa ng huge money.. Thank you!

Good day!
I have an inquiry lang po with regards to the info on my live birth.
My middle name is misspelled, instead na ELIZAGI dpat po ay ELIZAGA, pati po name ng mother ko ay misspelled, the correct one is THELMA ALVAREZ ELIZAGA ang nakasulat po doon ay TELHMA ALVAREZ ELIZAGA. My mother and father are not really married but somehow the person who processed the registration gave out info which are incorrect. I am planning on correcting my misspelled middle name, do I have to request as well for the correction on the info sa marriage nila to not married???
I am planning to get a passport po kasi, will I encounter any problem? Should I correct the error first before submitting an application? All my documents from TOR to info on my govt issued ID's I am using what is provided on my livebirth except sa name ng mother ko. I wrote the correct spelling everytime nagfifill out ako ng forms na ng.aask for my mothers name.

Natry ko na po yan.. wrong spelling din po ang middle initial ko sa birth certificate, Nung nagtanong po ako sa NSO, nilagyan nila ng statement sa right upper side corner ng birth certificate "Annotation" ang term nila... Tinanggap naman ng DFA, nakakuha naman po ako passport ko. Very Happy

Hi! Please advise me on this issue. I am applying for a passport. I already had an appointment to DFA when I relaized that my father's middle name in my birth cert. is incorrect. His correct middle name is SANTANA but on my bc, it's Sta. Ana. When I filled up the passport application form, I listed Santana bec. it is the correct one. (Im not holding my bc when I was filling up the passport app. form.) Should I apply anything to correct my birth cert.? What is annotation? Shall the dfa accept it as is? Thanks in advance.

Good day!
Atty, may problema po kasi sa middle name ko sa diploma at otr ko. Instead of Vigiga which is nakaregistered sa nso bc ko, Vegiga po ang nakalagay sa school record. I graduated in college last 2010 pa po. Hindi ko na po naipaayos agad kasi i got a job after i graduated. Now, i was planning to go abroad for work, and i think this one of the requirement. Please advice on how to fix this problem and ok lang po ba kung magsend ako ng correction letter directly sa CHED?
